Home from Hospital

Too many people stay in hospital longer than they need, just because they haven't got the right care and support at home.

 

That's why Ridgeway Care & Repair's Handihelp and Wiltshire Council have set up Home from Hospital, a new hospital discharge scheme for Wiltshire people.  

 

Our hospital discharge caseworker works with patients and hospitals, including the Great Western Hospital in Swindon and the Royal United Hospital in Bath, to make sure people have everything they need to return home.

 

A free service

It's a free service that provides practical help to help patients before and up to one month after they've returned home, such as home safety and heating checks, cleaning and shopping.

 

The scheme includes:

  • Making sure homes are clean and safe to return to

  • Stocking up with basic food supplies

  • Organising home heating and safety checks

  • Organising home care to help with washing, dressing, housekeeping, shopping, prescription collection and other tasks

  • Help with benefit claims or applying for grant funding

  • Organising minor home improvements and repairs, such as fitting Lifeline alarms, grab rails, level-access showers and stair lifts.

 

Who we can help

We're here to help people living in Wiltshire. excluding Salisbury, no matter what hospital they're staying in. Patients must be aged 60 or over, or at least 18 if they have a disability.

 

Age Concern is promoting the same service to Salisbury residents.
